I wish to participate in the Spectrum Sports Combat End of Year Adults Grading & Kit Dale Super Seminar (Brazilian jiu-jitsu, no-gi grappling and related training) at 7/50 Heaths Court, Mill Park VIC 3082 on Saturday 14 November 2026. I understand martial arts and combat sports involve physical contact, strenuous exercise, and inherent risks including but not limited to bruises, strains, sprains, fractures, concussion, and other injury. I voluntarily assume all risks associated with participation. I confirm I am physically able to take part, or I have cleared participation with a medical professional. I will tell the coach about any relevant injury, illness, or condition before training. To the fullest extent permitted by Australian law, I release and hold harmless Spectrum Sports Combat, its owners, coaches, staff, guest instructors and volunteers from claims arising from my participation, except where injury is caused by gross negligence. I consent to basic first aid if needed, and to Spectrum Sports Combat contacting my emergency contact listed in the form. I confirm I am 18 years or older.
